Pre-Authorized Payment Form (while in repayment)
Canada Apprentice borrowers who are repaying their loans can use this form to sign up for pre-authorized payments or to update their banking information.

This is a sample of a cheque marked as void. The pre-preprinted numbers at the bottom provide your account numbers. If there are four sets of numbers, the first set is the cheque number and not required. Of the next three sets of numbers:
- the first is the 5 digit Branch Transit Number;
- the second is the 3 digit Bank ID Number; and
- the third is the Bank Account Number.

Use this form if you want to apply for Repayment Assistance Plan for Borrowers with a Permanent Disability (RAP-PD) and would like your disability-related expenses or exceptional expenses to be considered.

Request to Opt Out of the Repayment Assistance Plan for Borrowers with a Permanent Disability
Use this form to request and provide consent to be processed for the Repayment Assistance Plan (RAP) instead of the Repayment Assistance Plan for Borrowers with a Permanent Disability (RAP-PD). You may only exercise this option prior to or within your first six month period of RAP-PD. This option is not available if you have already completed one or more period(s) of RAP-PD. To learn more about RAP and RAP-PD, and the future restrictions associated with each, please login to your Canada Apprentice Loan (CALSC) online service account at www.pca-cal.ca.
