Pre-Authorized Payment Form (while in repayment)

Canada Apprentice borrowers who are repaying their loan can use this form to sign up for pre-authorized payments or to update their banking information.

This is a sample of a cheque marked as void. The pre-preprinted numbers at the bottom provide your account numbers. If there are four sets of numbers, the first set is the cheque number and not required. Of the next three sets of numbers:
  • the first is the 5 digit Branch Transit Number;
  • the second is the 3 digit Bank ID Number; and
  • the third is the Bank Account Number.

Repayment Assistance Plan for Borrowers with Disabilities - Expenses Form

Use this form if you want to apply for the Repayment Assistance Plan for Borrowers with Disabilities (RAP-D) and would like your disability-related expenses or exceptional expenses to be considered.

Request to Opt Out of the Repayment Assistance Plan for Borrowers with Disabilities

If you no longer have a persistent or prolonged disability, select “No” at the question: “Do you attest that you currently have either a Permanent Disability, or a Persistent or Prolonged Disability?” when completing the RAP Application form and you will be assessed for RAP in place of RAP-D.

If you think you meet the eligibility criteria for borrowers with permanent disabilities, but would like to receive RAP in place of RAP-D, use this form to request and provide consent to be processed for the Repayment Assistance Plan (RAP) instead of the Repayment Assistance Plan for Borrowers with Disabilities (RAP-D). You may only exercise this option prior to or within your first six month period of RAP-D. This option is not available if you have already completed one or more period(s) of RAP-D. To learn more about RAP and RAP-D, please log in to your Canada Apprentice Loan (CALSC) online service account at www.pca-cal.ca.

Verification of Disability

If this is your first time applying for RAP-D and you have not received any PD or PPD grants, you will need to complete a Verification of Disability form to confirm your eligibility.

You can download the form by clicking on the Verification of Disability form link below. Once completed, please mail it to the Canada Student Financial Assistance Program at the address provided on the form.

Upload Forms

Upload your completed and signed form(s) and supporting documents through your Canada Apprentice Loan online service account. You will need to register for an account if you do not have one already. Alternatively, you can mail your completed form and documents to us.
